WHAT “SCARLESS” ACTUALLY MEANS AND WHY MOST SURGEONS DON’T OFFER IT

Many revision surgeons access the breast through the same incision used in the original surgery, often along the fold at the base of the breast or at the edge of the areola. Every revision adds to the scar burden on the breast itself.

Dr. Ching uses a different approach when patients are appropriate candidates. Using an advanced endoscopic technique, he makes a small incision concealed in the natural crease of the armpit. There is no incision on the breast, no new scar where it can be seen or felt.

WHAT MAKES THIS DIFFERENT FROM OTHER SURGEONS WHO USE AN ARMPIT APPROACH

The critical distinction is the endoscope, a small camera that lets Dr. Ching see directly into the surgical area throughout the procedure. Without it, armpit-incision surgery is performed with blunt instruments without direct visualization, which increases the risk of uneven dissection, bleeding, and asymmetry.

Dr. Ching has described surgeons who skip the endoscope as operating “essentially blind.” He is one of the few surgeons who have refined this endoscopic technique for revision cases — not just initial augmentation.

Dr. Shim Ching is a board-certified plastic surgeon based in Honolulu, Hawaii. He holds dual fellowship status from both the American College of Surgeons and the Royal College of Surgeons of Canada in plastic surgery, and is certified by the American Board of Plastic Surgery.
